Hazel is survived by her daughter, Carolyn Bowling and husband Gerald; sons, Leslie Woodard and wife Mendy and Wesley Woodard and wife Valissa; 11 grandchildren and 15 great-grandchildren.
Hazel is preceded in death by her loving husband Allen Woodard and son Lynn Woodard.
