Roy Menard Slezak

November 26, 1929 — December 11, 2016

Roy Menard Slezak, MD, age 87, peacefully passed away on December 11, 2016 at his home in Keller, TX. Born in Oak Park, Illinois in 1929, Roy was the second son of Vlasta and Gustav Slezak. He spent his childhood in Western Springs, Illinois, graduating high school in 1947. Roy attended college at William and Mary, graduating with a BS Pre-Med in 1951 and a M.D. in 1955 from the University of Illinois, College of Medicine. He worked his internship in Seattle WA and his OB-GYN residency at the Swedish Hospital and Madigan Army Hospital. Roy joined the US Army in 1957 and served at the Madigan Army and Kimbrough Army Hospital until 1962 as a Major Officer.

Roy began his career as an OB-GYN in Bowling Green, KY from 1962-1985 and was well respected by his patients and colleagues. He served as a board member of Bowling Green-Warren County Hospital and Greenview Hospital. He also served on the boards of Graves Gilbert Clinic, Kentucky Maternal Morality Review Board and the Tri-County Medical Society.

In 1985, Roy moved his medical practice to Salt Lake City, Utah where his reputation continued to flourish. He served on the boards of Pioneer Valley Hospital, Holy Cross Jordan Valley Hospital, and Premier Medical Network. Roy retired in 1997 and moved to St. George, Utah; however, he felt compelled to continue his mission of service to the medical community and worked as a Surgical Assistant at the Dixie Regional Medical Center until 2012.

Roy spent his final year at the Conservatory in Keller, Texas where he received excellent support by the staff and family members. He remained active in community events and made many friends who referred to him as “Doc.”

Memories of Roy include his humor, quick wit, and dedication to his work and family. He enjoyed golfing, tennis and playing bridge. He touched so many lives, delivered countless number of babies into the world and will be missed by his many wonderful friends and family. Roy is preceded in death by his parents and the love of his life and wife of 23 years, Patricia E. Slezak. Roy is survived by his brother, George Slezak, his 7 children: Karen Christensen (Marty); Patricia Talley (Steve); Jeff Slezak; John Slezak; Charles Slezak (Celine); Tracy Beatty; and Joseph Morton, 17 grandchildren and 6 great grandchildren.

Roy has requested a private memorial service with immediate family.
